			<content:encoded><![CDATA[<p>A new public plan — to offer consumers greater choice, keep the private plans honest and, one can hope, restrain the relentless growth in health care premiums and underlying medical costs — seems worth trying.</p>
<p>Any new public plan would constitute only part of a much broader effort to provide coverage for 46 million Americans who currently are uninsured and many more who may soon join their ranks. Other major parts under discussion include strengthening employer-provided coverage, expanding existing public programs such as Medicaid and creating a national health insurance exchange where individuals without employer coverage, small businesses and possibly others could buy policies at inexpensive group rates from qualified private plans and, we hope, from a new public plan as well.</p>
<p>Many reformers suggest that a public plan be modeled on Medicare. If crafted correctly, it would provide a valuable option for people who don’t trust private insurers to have a patient’s interest at heart and would offer a safe haven should private plans abandon a market, leaving their subscribers stranded. It would also serve as a competitive yardstick for measuring the performance of private plans.</p>
<p>A public plan might do a better job of slowing the growth of health care costs, although Medicare has not been notably successful in that regard. The public plan would almost certainly have lower administrative costs. And it could probably force doctors and hospitals to accept lower reimbursements than they negotiate with private insurers, allowing the public plan to charge lower premiums and attract more customers.</p>
<p>The insurance industry and leading Republicans decry that as unfair competition. Hospitals and doctors worry about being underpaid, but there is a limit to how low government reimbursements can fall without driving them out of the program.</p>
<p>What many critics seem to fear most is that a new public plan would sweep away its private competitors and evolve over time into a full-fledged single-payer system (sometimes called Medicare for all). No matter how fair the competition between public and private plans might be at the start, they warn that the government would find it irresistible to rig the outcome through its regulatory and pricing powers and its ability, in a pinch, to subsidize the public plan with taxpayers’ money.</p>
<p>That fear seems overblown. Innovative, nimble private plans with well-integrated service systems might outperform any government plan, just as some now outperform Medicare through better coordination of services, stronger preventive care and broader benefits.</p>
<p>A new public plan is neither the cornerstone of health care reform nor the death knell of private insurance. It should be tried as one element of comprehensive reform. If, over time, a vast majority decides the government plan is superior, so be it. </p>

			<content:encoded><![CDATA[<p>While the above headline may not seem that shocking… Here is the real kick in the head. 75% of those personal bankruptcies… had health insurance!</p>
<p>That’s right. Hardworking families… doing what they do, making a living, paying their bills (including health insurance). And when the big day came they were just not really protected. I understand, no one can necessarily predict the future. No one knows what the future may hold.</p>
<p>That is why choosing a health insurance plan can be such an unpleasant experience sometimes. So I approach it from a different angle. Working for a non-profit my job is to ensure you get the right plan for you.</p>
<p>But no matter if you are talking to me or another person offering health insurance, just be sure you get the right plan.</p>
<p>Here are some things to consider:</p>
<p>1. Understand your insurance plan and your out-of-pocket exposures. This can be easily glossed over in the presentation. Ask tough questions, run through ALL the possibilities and make sure you understand it. If you don’t… do not feel bad. Make the person explain it until you do.</p>
<p>2. Check cancellation clauses. Kind of an obvious one. But sometimes this point is not totally understood.</p>
<p>3. If you are on a “group insurance plan” through your company consider benefits, price, options as well as out-of-pocket exposure to a plan you can just as easily obtain on your own. You might be surprised.</p>

<p>Let’s look at the facts:</p>
<p>Lets’ say that an individual goes uninsured to he/she reaches thirty. At that point he/she gets married and starts a family. Now 32 with a baby on, the way the couple decides to purchase health insurance. With a yearly family deductible for 3 family members, in their age bracket, the family will pay a insurance premium of about $800.00 per month ($9,600 per year). The first $1,000 (deductible) comes out of their pocket, followed by a $20 co-pay for office visits and a 20% co-pay on all procedures.</p>
<p>Believe it or not, the average healthy American may never use their insurance, other than for their routine yearly physical. If that’s the case than they will never meet their yearly deductible and the entire visit will be out-of-pocket. Example: the first year the family (above) who is having a baby will pay $9,600 for the year of insurance, $1,000 for the deductible (for having the baby – all visits and birth), and after the deductible is satisfied, then 20% co-pays for all visits and procedures. In the first year it will cost the family over $11,000 to have health insurance. Note: I paid cash for all midwife visits, ultrasounds, blood tests and birthing. The total cost for my daughter to be delivered was $6,500,</p>
<p>The baby is now born and it’s a new year’s worth of premiums. This family is otherwise healthy. The family takes proactive actions to take care of themselves. They get their teeth checked once a year ($100 each), husband and wife both get a yearly physical with blood tests. ($400-600 for both), and the baby gets well-baby visits and immunizations ($400). At this point the deductable has been met but the insurance is not needed for the remainder of the year. Yearly cost: with slight premium hike its $11,000.</p>
<p>Over the next four years their insurance premium goes-up slightly. The family is healthy, a few colds, maybe a visit here and there for routine check-ups, but not enough to meet the yearly deductable, so it’s all out-of-pocket. In that four year period they spend an estimated $47,000 on health insurance.</p>
<p>The couple is now 38 years of age. They have spent about $70,000 to have health insurance and have only recouped about $10,000 worth of received benefits. This is a net loss of $60,000 and the couple is only 38 years old.</p>
<p>Assuming the couple eats right, goes to the gym, gets a few chiropractic adjustments, gets routine physicals with blood tests and doesn’t get hit by a car, than the chance of them utilizing their health insurance for anything major will be very slim. The sad part is they are paying for insurance, a peace of mind, that god-forbid if anything happens they will be covered.</p>
<p>Another ten years goes by, the insurance premiums go up a bit and the total expenditure for the ten years is over $110,000. In sixteen years this couple has spent over $180,000 to have insurance and has only utilized $10,000 worth of benefits, a net loss of $170,000. But, they did have peace of mind.</p>

<p>The Private Health<a href="http://www.ampminsure.org/"> Insurance</a> Administration Council (PHIAC) has produced a booked let Insure? Not Insure? that is available in html version and Pdf (455kb). This document has a useful list of questions to ask your insurer such as:</p>
<ul>
<li>Will my hospital cover provide benefits for all procedures or types of treatment?</li>
<li>Which hospital treatments will not be covered by this policy?</li>
<li>Is ambulance cover included with my hospital cover?</li>
<li>Which operations are considered elective?</li>
</ul>
<p>The website for researching private health insurance products is run by the private health insurance ombudsman&#8217;s office, and consumers associated with the Consumers&#8217; Health Forum assisted that office to make the website as user friendly as possible.</p>
<p>The Consumers Health Forum of Australia has completed work on private health insurance with a focus on key initiatives under the private health insurance reforms. These initiatives include: informed financial consent arrangements; the consumer information website administered by the Private Health Insurance Ombudsman; the introduction of broader health cover; and the review of prostheses listing arrangements.</p>
<p>One of the issues affecting consumers we identified last year was ambulance cover. ACT residents do not receive free ambulance service unless they hold an ACT Pensioner Concession or Health Care card. ACT residents are however, covered for ambulance services provided within the ACT as a result of a road traffic accident – through the road rescue fee levied on vehicle registration. Medicare does not cover the cost of ambulance services. Private health insurance policies usually cover ambulance services – but we suggest you check with your health fund.</p>
<p>The Health Minister Roxon was very clear at the recent AHCRA summit that the Private Health Insurance rebate is here to stay but we wonder whether given the current budget issues and the renewed call for need to address middle class welfare how long the government can sustain this. We will be interested to see the federal budget.</p>
<p>With the private health insurance rebate it stops being only a health decision but rather becomes more of an economic one. However the financial benefits only outweigh the direct costs for those people who are medium to high income earners.</p>
